Yes, Wegovy (semaglutide) is available on the NHS in the UK — but only through specialist weight-management services, and only for people who meet strict criteria. Access is limited, waiting lists are long, and many people who want it will not qualify under the current NHS rules.
This guide explains how NHS access to Wegovy actually works, who is eligible, why it can be hard to get, and what your options are if the NHS route isn't open to you.
The short answer
Wegovy is approved by NICE (the National Institute for Health and Care Excellence) for weight management on the NHS. However, it is prescribed within specialist weight-management services, not routinely by your GP as a first step. Because these services have limited capacity, access is tightly controlled and often involves a wait.
In practice, that means many people who are clinically suitable for Wegovy still cannot obtain it quickly through the NHS. Understanding the eligibility rules helps you see where you stand.
Who is eligible for Wegovy on the NHS?
NHS access to Wegovy generally requires that you are referred to a specialist weight-management service and that you meet body mass index (BMI) thresholds alongside a weight-related health condition. The typical NICE criteria include:
- A BMI of at least 35 with at least one weight-related health condition (such as type 2 diabetes, high blood pressure or obstructive sleep apnoea), or
- In some cases a BMI of 30–34.9 where you meet the criteria for referral to a specialist service.
Lower BMI thresholds may apply to some ethnic groups where health risks occur at a lower BMI. Your clinician will assess this individually.
Crucially, NHS Wegovy is also intended to be time-limited in many services and is provided alongside a structured programme of diet, physical activity and behavioural support — not as a standalone prescription.
Why is it so hard to get on the NHS?
There are a few reasons why NHS access is more limited than many people expect:
- Specialist-service gatekeeping. Wegovy is delivered through tier-3 and tier-4 weight-management services, which have finite capacity and can carry long waiting lists.
- Referral requirements. You usually need a GP referral into these services, and not everyone who asks will be referred.
- Supply and prioritisation. Availability has fluctuated, so services often prioritise those with the highest clinical need.
None of this means Wegovy is unsafe or ineffective — it simply reflects how a limited NHS resource is rationed. It's the same medicine whether it's prescribed on the NHS or privately.
What is Wegovy and how does it work?
Wegovy is a GLP-1 receptor agonist — it mimics a natural gut hormone that helps control appetite. You feel fuller sooner and for longer, and the stomach empties more slowly, which supports eating less without constant hunger.
It was the first medicine to be MHRA-licensed specifically for weight management. The Wegovy injection is a once-weekly self-injection, titrated gradually from a low starter dose upward to reduce side effects.
In the STEP UP clinical trial, adults on the 7.2mg dose lost on average 20.7% of their body weight over 72 weeks, alongside lifestyle support. Individual results vary; not everyone reaches this.
Common side effects
As a GLP-1 medicine, the most common side effects (affecting more than 1 in 10 people) include nausea, diarrhoea, constipation, vomiting, decreased appetite and fatigue. These are often worst when starting or increasing a dose and usually settle over time.
Rare but serious risks include severe allergic reaction, acute pancreatitis, severe hypoglycaemia and severe gastrointestinal disease. If you experience a side effect, you can report it via the MHRA Yellow Card scheme at https://yellowcard.mhra.gov.uk/.
The private route: what to expect
If you don't qualify for NHS Wegovy or can't wait, a regulated private online pharmacy is a legitimate alternative. The key is choosing a service that is properly regulated in the UK — not an overseas or grey-market seller.

Wegovy is one of several licensed weight-management medicines. Depending on your assessment, a prescriber might discuss:
- Wegovy Tablet — the same active medicine (semaglutide) as the injection, taken as a once-daily tablet on an empty stomach with a small sip of plain water, waiting at least 30 minutes before eating or drinking anything else. In the OASIS 4 trial, adults on oral semaglutide 25mg lost on average 16.6% of their body weight over 64 weeks, alongside lifestyle support. Individual results vary; not everyone reaches this.
- Mounjaro (tirzepatide) — a weekly injection that activates two gut-hormone receptors (GLP-1 and GIP). In the SURMOUNT-1 trial, participants on the 15mg dose lost on average 22.5% of body weight over 72 weeks, alongside lifestyle support. Individual results vary; not everyone reaches this.
