Foundayo is Eli Lilly's orforglipron — the first GLP-1 tablet built as a small molecule rather than a peptide. That is the reason it can be swallowed at any time of day, with or without food or water, where other oral GLP-1s need an empty stomach and a waiting period. We prescribe it after a full clinical review and dispense from our own in-house pharmacy.
A GLP-1 that survives digestion.
Orforglipron is a GLP-1 receptor agonist — it mimics a natural gut hormone that helps regulate appetite, slows how quickly the stomach empties so you feel fuller for longer, and improves blood-sugar control after eating. That much it shares with Mounjaro, Wegovy and the Wegovy Tablet.
What is different is its chemistry. The other GLP-1 medicines are peptides — proteins, which the gut digests. An injection bypasses that problem; the Wegovy Tablet works around it with an absorption enhancer, which is why it must be taken fasted with a small sip of water and a 30-minute wait. Orforglipron is a small molecule, not a peptide, so it is not broken down in the same way and needs neither trick. It is an ordinary tablet.
In practice that means one tablet a day, swallowed whole, at whatever time suits you — with food or without, with water or without. For a lot of people that is the deciding difference, because a medicine you take reliably works better than one you take around a fasting window you sometimes miss.
Foundayo is MHRA-authorised for weight management and type 2 diabetes in adults, granted on 10 August 2026. It is a prescription-only medicine and must be prescribed by a UK-registered clinician after proper assessment.
Start low. Step up slowly.
Everyone starts on 0.8mg — including people switching from another GLP-1 — and moves up no sooner than every 30 days, each step reviewed by your prescriber. Six strengths exist, but most people settle well below the top of the ladder.
0.8mg → 2.5mg
- 0.8mg is a starting strength, not a treatment dose
- It gives your body time to adapt before the strength rises
- Most people move up after about four weeks
- Both strengths dispatch now
5.5mg → 9mg
- Minimum 30 days on the previous strength
- Your prescriber reviews tolerability before each step
- Staying longer at one strength is a normal choice

- 17.2mg is the maximum licensed daily dose
- Never more than one tablet a day to reach a higher dose
- Any of the upper strengths may be your maintenance dose

What you might experience.
Foundayo has the GLP-1 class side-effect profile — most are gastrointestinal, most noticeable when starting or increasing the dose, and usually settling over a few weeks. This isn't exhaustive — always read the patient information leaflet provided with your medicine.

- Severe hypoglycaemia
- Gallbladder disease
- Worsening diabetic retinopathy
Foundayo is a Black Triangle medicine, meaning the MHRA is collecting extra safety information on it. If you experience a side effect you can report it directly through the MHRA Yellow Card scheme, and please tell us as well so we can record it and advise you.
Who can — and can't — take this.
Foundayo is a powerful licensed medicine, and like any powerful medicine it isn't right for everyone. Your clinician assesses your full history during consultation.
Foundayo may be suitable if you…
- Are aged 18 or over
- Have a BMI of 30+ (obesity)
- Have a BMI of 27–30 with a weight-related condition (e.g. type 2 diabetes, high blood pressure, sleep apnoea)
- Would rather take a tablet than inject
- Have found the fasting window on other oral treatments hard to keep to
- Are ready to support treatment with diet and activity changes
Foundayo is not suitable if you…
- Are pregnant, planning pregnancy, or breastfeeding
- Have type 1 diabetes
- Have a personal/family history of medullary thyroid carcinoma or MEN2
- Have a history of pancreatitis
- Have severe gastrointestinal disease (e.g. gastroparesis)
- Have an active eating disorder
- Have severe liver or kidney impairment (case-by-case)
- Are allergic to orforglipron or any excipient
