Cold-Chain Delivery of Weight-Loss Injections: How Mounjaro and Wegovy Reach You Safely

When you order a weight-loss injection online, you are not really buying a product in a box. You are buying a peptide medicine that only works if it stays cold from the moment it leaves the pharmacy fridge to the moment it reaches yours. Both Mounjaro (tirzepatide) and Wegovy (semaglutide) are stored at 2°C to 8°C, and how they travel matters as much as what is inside the pen. This guide explains how the cold chain actually works, what to check when your parcel lands, how to store your pen once it arrives, and how to tell a regulated pharmacy delivery from a risky one.

Both medicines are prescription-only. They are supplied in the UK only after an online consultation and a clinical assessment by a prescriber — never sold "over the counter" or shipped without a proper review. That clinical step is also the first line of your safety, because it is the point at which a genuine pharmacy checks that the medicine is right for you and that the supply chain behind it is legitimate.

Why weight-loss injections have to stay cold

Tirzepatide and semaglutide are peptides — short protein-like molecules. Proteins are delicate. Heat, freezing and sometimes light can change their structure, and a peptide that has degraded may be less effective. This is why the manufacturers and the Medicines and Healthcare products Regulatory Agency (MHRA) set a defined storage range rather than leaving it to chance.

For an unopened pen, the licensed instructions are clear:

Two points people often get wrong. First, freezing is as damaging as overheating — a pen that has frozen must not be used, even if it looks fine. Second, that room-temperature allowance is a use-it-up window once the pen is in your hands, not a licence for a courier to leave a parcel baking in a hot van for three days. An unopened pen should stay in the fridge at 2°C to 8°C; the 30-day (Mounjaro) window applies once you have taken the pen out and started using it. Every hour a pen spends warm in transit is an hour eaten out of that in-use window before you have even started. A properly run delivery protects the fridge temperature all the way to your door so you get the full shelf life.

Always read the patient information leaflet inside your specific pack, as storage details can differ slightly between brands, pens and batches.

How a cold-chain delivery is actually packed

A medicine that must stay at 2°C to 8°C cannot travel in an ordinary padded envelope. A regulated pharmacy ships it in insulated packaging built to hold temperature for the length of the journey:

• Insulated outer packaging — high-density insulation (often wool-based or foil-lined liners) rather than plain cardboard, chosen to buffer against summer heat and winter cold.
• Medical-grade coolant packs — gel packs conditioned to keep the air around the pen cold without freezing it. Supermarket freezer blocks are not the same; a badly packed box can freeze the medicine and ruin it.
• A sealed, protected pen — kept in its original carton so it is shielded from light and physical damage.
• Tracked, often signed-for transit — so the parcel is handed over or left somewhere safe rather than lost for days.

The aim is simple: keep the inside of that box within range for the whole journey, typically designed to hold for 24 to 48 hours to cover normal transit and a missed first attempt.

What to check the moment your parcel arrives

Open it promptly and do a 30-second inspection before anything goes in the fridge:

1. Feel the coolant packs. Still cold or gel-like? Good. Warm and fully liquid after a long delay is a reason to pause.
2. Feel the pen. It should feel cool. It should not be warm, and it should not be frozen solid.
3. Look at the liquid through the inspection window. Genuine Mounjaro and Wegovy solution is clear and colourless to slightly yellow. Cloudiness, particles or discolouration means do not use it.
4. Check the pen and carton for cracks, leaks or damage.
5. Check the expiry date and that the brand/strength match your prescription.

If anything looks wrong — a warm pen after a long delay, cloudy liquid, a cracked pen — do not inject it. Contact the pharmacy the same day for a clinical safety check. A responsible pharmacy would far rather assess a questionable pen than have you use one that may have been compromised. If you do inject and then experience a suspected side effect, you can report it through the MHRA Yellow Card scheme at yellowcard.mhra.gov.uk, which helps monitor the safety of all UK medicines.

Storing your pen once it is home

Get it into the fridge quickly, and store it well:

• Put it in the main body of the fridge, not the door — the door swings between warm and cold every time it opens.
• Keep it away from the back wall and the freezer compartment, where things can accidentally freeze. Frozen tirzepatide or semaglutide is no longer usable.
• Leave it in its original carton to protect it from light.
• Once you start using a pen and keep it out of the fridge, note the date — you have a limited number of days (up to 30 for an in-use Mounjaro pen kept below 30°C, and up to 28 for Wegovy — check your leaflet) to finish it, after which it must be discarded even if solution remains.

Discreet packaging is a safety feature, not just privacy

Legitimate weight-loss medication is delivered in plain, unbranded outer packaging. There is no clinic name, no "weight loss", no drug name printed on the outside. That protects your privacy — your treatment is nobody else's business — but it is also a security measure. High-value medicines are a target, and an anonymous box is far less likely to be tampered with or taken than one advertising what is inside.

If you use a nominated safe place, make sure it is genuinely shaded and secure. A temperature-sensitive parcel left in a sun-trap porch or a plastic bin on a warm afternoon can overheat long before you get home to it.

Missed deliveries, heatwaves and holidays

The cold-chain packaging is generous, but it is not infinite. Most insulated shippers are designed to hold temperature for roughly 24 to 48 hours, which comfortably covers normal transit and a single missed attempt. If you miss the first delivery, arrange redelivery quickly rather than letting the parcel sit at a depot over a long weekend.

Timing your order around your own life helps too. If you are about to go away, it is usually better to have your pen delivered when you will be home to move it straight to the fridge, rather than have it arrive to an empty house in the middle of a heatwave. If you are travelling with a pen, carry it in an insulated bag with a cool pack (not touching the coolant directly, so it cannot freeze), keep it out of hot cars and direct sun, and get it back into a fridge as soon as you can. When in doubt about whether a pen has stayed within range, ask the pharmacy before you use it.

The bigger risk: unregulated "grey market" sellers

The cold chain only means something if what is inside the pen is real. This is where buying from a regulated UK pharmacy matters most.

The MHRA has been running major enforcement against illegal weight-loss injections. In 2025 it disrupted illicit manufacturing sites in England — including a raid on an unlicensed operation in Northampton in October 2025 — and an ITV News investigation that year found pens sold as tirzepatide that actually contained semaglutide at many times the recommended starting dose, plus warnings that some fakes contained contaminants such as insulin — which, in someone without diabetes, can cause dangerous, even life-threatening low blood sugar. If you ever suspect a bad reaction from any medicine, genuine or not, report it to the MHRA via the Yellow Card scheme at yellowcard.mhra.gov.uk. Sellers who skip the prescription and the clinical check are exactly the ones who also skip the cold chain and the quality controls behind it.

The warning signs of an unsafe source are consistent:

• No prescription required and no clinical assessment before purchase.
• No pharmacist or prescriber involved, and no GPhC-registered pharmacy details you can verify.
• Medicine arriving in a plain bubble envelope with no cold-chain packaging.
• Prices that look too good to be true, and social-media or messaging-app "sellers".

A genuine UK online pharmacy is registered with the General Pharmaceutical Council (GPhC), dispenses only after a prescriber has reviewed your case, and ships within a real cold chain. If any of those three things is missing, walk away.

It is worth being just as wary of a stranger offering "spare" pens through social media or a friend passing on medication they no longer need. Even if the pen is genuine, you have no way of knowing how it was stored — whether it was left in a warm car, frozen at the back of a freezer, or kept past its in-use window. Prescription weight-loss medicine is prescribed to one named person after an individual assessment, and a pen with an unknown storage history is not something you can trust, however well-meaning the offer.

A note on safety and what to expect

Weight-loss injections are effective medicines, but they are still medicines. The most common side effects are gastrointestinal — nausea, diarrhoea, constipation and reduced appetite — which is one reason both are started at a low dose and increased slowly. Mounjaro, for example, starts at 2.5mg once weekly and is increased by 2.5mg no sooner than every four weeks, up to a maximum of 15mg, to help your body adjust.

Rarer but more serious risks include pancreatitis (severe, persistent tummy pain, sometimes spreading to the back) and gallbladder problems. These medicines are not suitable for everyone, including in pregnancy or breastfeeding and in certain medical conditions, which is exactly why the prescriber assessment exists.

In terms of what they can achieve, in the SURMOUNT-1 trial, tirzepatide 15mg produced an average of around 22.5% body-weight loss over 72 weeks alongside diet and activity, and in the STEP-1 trial, semaglutide 2.4mg produced around 14.9% over 68 weeks. These are trial averages — individual results vary, and they were achieved with lifestyle changes, not from the injection alone.

Both medicines are licensed by the MHRA for weight management for adults with a BMI of 30 or more, or 27 or more with a weight-related health condition (such as high blood pressure or type 2 diabetes).
