How to Store Your Mounjaro Pen When Travelling Abroad

A Mounjaro pen packed in a small insulated cool bag with an ice pack, ready for air travel

Keep your Mounjaro (tirzepatide) pen cool, in your hand luggage, and out of direct heat. Before first use, store pens in a fridge at 2–8°C; once in use, a pen can be kept below 30°C and used for a limited time — so an insulated cool bag with a cold pack (never a pen pressed directly against ice) is the safe way to carry it through airports and on holiday.

Travelling with an injectable weight-loss medicine is very common and entirely manageable with a little planning. This guide explains the temperature limits, how to pack a cool bag, airport and hand-luggage rules, and what to do if things go wrong.

The temperature rules that matter

Always follow the storage instructions in your specific pack's Patient Information Leaflet, as these are the authoritative source for your product. In general terms:

  • Unused pens: keep refrigerated at 2–8°C until you are ready to use them. Do not freeze — a pen that has frozen must be thrown away, even if it looks fine.
  • In-use / out of the fridge: once removed from refrigeration, a pen can usually be kept at room temperature (below 30°C) for a limited number of days, after which it must be used or discarded. Check your leaflet for the exact window.
  • Keep it out of direct sunlight and heat — a hot car, a sunny windowsill, a poolside bag or a beach towel can all push the pen above its limit quickly.

The reason temperature matters is that tirzepatide is a biological medicine. Excess heat or freezing can degrade it, which may make it less effective — and you cannot always tell by looking.

Choosing and packing a cool bag

For journeys longer than a couple of hours, a small insulated medical cool bag is the simplest solution. Look for one designed for insulin or diabetes supplies, as these are the right size and are widely airline-accepted.

How to pack it safely

  • Use a gel cold pack rather than loose ice. Loose ice melts into water, and a leaking bag is a problem in hand luggage.
  • Never let the pen touch the cold pack directly — wrap the pen in a cloth or use a bag with a separate compartment, so it stays cool without accidentally freezing.
  • Keep the pen in its original carton for protection and so the label, batch and expiry are visible.
  • Pack enough pens for your trip plus a little spare in case of delays, and keep needle tips and sharps arrangements with them.

For very long trips, specialist travel wallets that stay cool for 24–48 hours (some are activated with water rather than a freezer) can be useful, especially where you cannot rely on a fridge at the other end.

Airport security and hand luggage

Always carry your Mounjaro pens in your hand luggage, never in the hold. The hold is unpressurised for temperature and can drop below freezing or get very warm, and checked bags can be lost or delayed — leaving you without your medicine.

Getting through security

  • Medicines and medical cool bags are allowed through UK airport security even though they may exceed the usual 100ml liquid rule. Tell the security officer you are carrying a prescription medicine that needs to stay cool.
  • Carry a copy of your prescription or a letter confirming the medicine is yours and why you need it. If you are a Weight Clinic patient, you can request supporting documentation via our contact page before you travel.
  • Needles are permitted in hand luggage when accompanied by the injectable medicine and documentation — keep them in the original packaging.
  • Frozen gel packs can occasionally be questioned at security. A gel pack that is partly thawed (soft rather than rock-solid) is usually smoother to process; you can re-freeze or re-chill it once airside.

Rules can vary by country

Entry rules for carrying prescription medicines differ between countries, and a few have strict requirements for certain drugs. Before you fly, check the entry requirements for your destination (and any countries you transit through) on official government travel advice, and carry your prescription paperwork to be safe.

Time-zone changes also affect your once-weekly injection timing. Mounjaro is taken on the same day each week and can be given at any time of day, which gives you flexibility — if your usual injection day falls during travel, you can keep to the same day in your destination's local time. If you are unsure, speak to your prescriber before you go.

At your destination

  • Use a fridge where possible. Hotel mini-bars can run colder or warmer than a standard fridge, so avoid placing the pen against the back wall or coldest shelf where it could freeze. If in doubt, a mid-shelf position is safer.
  • No fridge? A pen already in use can stay below 30°C for a limited time per the leaflet — keep it in your cool bag, shaded and away from heat, and plan to use or discard it within the stated window.
  • Hot climates and beaches: never leave the pen in a parked car, a sunny room, or an unshaded bag. Heat is the single biggest risk on a summer holiday.

If a pen gets too hot, frozen, or you run short

If a pen has frozen, or you suspect it has been exposed to excess heat, do not use it — degraded medicine may not work properly. If you are left short because of a delay or a discarded pen, do not try to buy a substitute from an unregulated source abroad.

Instead, contact your prescriber or pharmacy for advice. The Weight Clinic dispenses only genuine, MHRA-licensed medicines from our own GPhC-registered UK pharmacy, so you always know what you are getting — you can read more about us or get in touch via contact.

Side effects while travelling

Common side effects of Mounjaro include nausea, diarrhoea, constipation, vomiting, decreased appetite and fatigue. These can be more noticeable when your routine, food and hydration change on holiday, so take care to stay hydrated, particularly in hot weather.

Rare but serious effects include severe allergic reaction, acute pancreatitis, severe hypoglycaemia and severe gastrointestinal illness — seek medical help if you are concerned. You can report suspected side effects through the MHRA Yellow Card scheme at https://yellowcard.mhra.gov.uk/.

Mounjaro is a prescription-only medicine. In the SURMOUNT-1 trial, participants on the 15mg dose lost on average 22.5% of body weight over 72 weeks, alongside lifestyle support (Jastreboff AM, et al. N Engl J Med. 2022;387(3):205–216). Individual results vary; not everyone reaches this.
